Vacuum assisted closure (V.A.C.®) Therapy has helped to improve outcomes and has led to a number of dramatic improvements in clinical practice. While V.A.C.® Therapy has been integrated into wound healing and is widely used around the world, some aspects of its use have yet to be standardized for optimal wound care outcomes. This World Union of Wound Healing Societies initiative describes when and how to successfully integrate the V.A.C.® technology into clinical practice, focusing on six key wound types. The content is based on the consensus opinion of an international group of experts. It is designed to be practical and adaptable for local use in countries worldwide. The latest clinical and economic evidence pertaining to the optimal use of V.A.C.® Therapy will be incorporated and discussed during this session.

Wound-related pain has a major impact on patients' quality of life. Pain at dressing changes is a well recognized problem which has gained an increased awareness in recent years. The satellite symposium objective is to provide the latest in managing wound patients' pain with new tools available, including presentations on new evidence on pain during dressing removal with products with Safetac technology. The satellite symposium will also present advise on how clinicians can make a difference regarding wound pain in the daily practice. New results of a large multinational survey in patients with chronic wounds will be presented. In order to support clinicians in their every day practice, WUWHS will launch a new guide with recommendations that facilitates best practice regarding minimising pain at dressing related procedures during the congress. This new best practice document will be presented during the symposium.

Compression is the cornerstone of leg ulcer management, yet there is a wide variatrion in the delivery of existing care. Clinicians are challenged with resource limitations. This is compounded by a wide variety of products which often require a high level of skill to apply effectively. There is a clear need of evidence based practical guidelines on the use of compression to assist clinicians to deliver effective care. This symposium launches a WUWHS best practice initiative based on the concensus opinion of an international group of experts. It focuses on the science of compression, emphasizes important points for clinical practice and discusses challenges and future developments in this area, all of which is applicable for use by clinicians worldwide.

This symposium presents a new WUWHS Consensus Document on a significant type of wound pain, the persistent (chronic) wound pain. Persistent wound related pain is a common experience that requires appropriate assessment and treatment. It is no longer adequate to concentrate on the pain during dressing change alone.
The consensus document provides new insights into patient-centered concerns, the origins of wound pain and best clinical practice principles for the assessment and management of wound pain.
The symposium will include aspects of assessing and documenting wound pain and patient centered concerns. It will differentiate wound pain in relation to its origin whether it is due to tissue damage, local infection, dressing change related procedures and provide guidance on how to effectively manage the wound pain with different origins. A safety review on topical use of ibuprofen and the results from an 853-patient randomized, comparative study on a foam dressing with ibuprofen, Biatain Ibu, will be presented.

This Symposium is designed to align the multidisciplinary approaches to the management of wound infection. It will attempt to describe both interventional and management strategies associatied with at risk and infected wounds from a clinician, nurse and minrobiologist's perspective. The intended focus will be on both acute and chronic wounds and the approaches described will help give guidance which is both practical and adaptable for clinical settings worldwide. The symposium will address definitions associated with diagnosing infection and the increased bacterial burden in a wound giving further clarity to terms such as critical colonisation. Recommendations on when to start treatment will be given along with advice on what time periods following commencement reassessment of the wound should occur. Optimisation on sampling techniques will be compared to the level of accuracy associated with the microbiological findings. At the same time recognition of how practical and applicable these techniques are in various care settings will be discussed, prioritisation of infection management goals will be highlighted together with other patient centered concerns such as pain management. All presenters will attempt to provide clarification of the challenges associated with this common problem and attempt to provide solutions that can be translated into everyday clinical practice.

ABSTRACT: Nutrition plays an important role in wound healing, repair of soft tissue injuries and recovery from surgery. Research has demonstrated that nutritional state and wound healing are strongly associated. Collagen synthesis is required for complete wound healing and resolution of tissue injury. Although the wound healing process is complex, the ultimate outcome of any healing process is repair of a tissue defect. Control of wound collagen synthesis has been an indefinable goal, and in many clinical instances, the goal is to increase the collagen deposition as a means of enhancing wound strength and integrity. Recent research supports the use of specific nutrients in order to enhance collagen deposition at the wound site. Specific amino acids, and their derivatives may also be beneficial for healing.

OBJECTVE: The seminar has been organized as a tribute to the pioneering work of Thomas Hunt by highlighting new insights to the role of oxygen and lactate in wound healing. It will also highlight the exciting potential of new molecular diagnostic and theranostic techniques in to guide the cost effective management and therapy of wounds. ABSTRACT: Many factors are known to influence the rate of healing or the transition from a non-healing to a healing state. The problem for clinicians is deciding which, if any, of the known factors they can influence with their present tool-kit. Some of these factors are biochemical and oxygen balance is an example of one that is of crucial importance but very difficult to assess. New tools are now emerging that allow clinicians to intervene practically and simply with programmed oxygenation and lactate control (e.g. Oxyzyme and Iodozyme). The significance of these in clinical practice will be described. As an example, it has been found that lactate in the presence of oxygen can initiate angiogenesis and connective tissue synthesis, while sustained hypoxia actually inhibits neovascularisation. Evidnece that lactate indirectly enhances endothelial cell migration by stimulation of VEGF production will be reviewed. But there is still the problem for clinicians of knowing when to intervene with therapies that influence or control such "invisible" biochemical factors, and of knowing when the therapies are having the desired effect. That's why the profession needs objective and simple molecular diagnostic tests. The potential and scope of wound diagnostic tests will be described and set in the context of the needs of clinicians at the point of care.

The underlying abnormalities of difficult to heal and chronic wounds (such as diabetic, venous, and pressure ulcers) promote the continuous deposition of fibrotic and necrotic debris that can provide an substrate for infection and interfere with granulation and re-epitelialization. There is a growing consensus that such wounds require regular maintenance debridement in order to keep the wound bed free of this debris and maintain the readiness of the wound for healing. This session will discuss the impact of maintenance debridement on different aspects of the healing process and explore a variety of issues related to the successful incorporation of maintenance debridement into an effective wound care strategy.

Patients with chronic wounds require holistic care from the hospital to home including the treatment of the cause and attention to patientcentered concerns. Local wound care has three main components: debridement, inflammation or infection control and exudate management. Excess exudate can lead to maceration, increased incidence of infection and deterioration of chronic wounds. The use of V.A.C.® Therapy in the local treatment of exudate can improve local wound care and accelerate healing.
In this session, we will review successful outcomes from the use of V.A.C.® Therapy in the homecare setting by healthcare professionals, patients and healthcare systems. Todd Nicholson, the Canadian Paralympic sledge team captain will tell us about his use of V.A.C.® Therapy as part of his gold medal success story. This symposium will highlight the safety of V.A.C.®, Therapy in the community and homecare setting. To improve clinical outcomes, we have linked education to scientific evidence, expert knowledge, and patient preferences.

INTRODUCTION: Soft tissue injury is often painful and prolonged and can have a major impact on a person's quality of life.
PURPOSE AND OBJECTIVE: Phototherapy (use of light) has played a major role in accelerating tissue repair and controlling pain in rehabilitation and sport medicine conditions, dental applications and wound infections. Recent studies have shown its value in the treatment of both acute and chronic wounds. The results have demonstrated how phototherapy improved healing and reduced costs. Another component of wound management is monitoring of the wound's progress. Effective and efficient monitoring of several wound parameters can assist with wound healing.
METHOD: Comprehensive literature searches identify the positive effects of phototherapy in wound healing and isolated a number of research based parameters for monitoring wounds. A wound healing trial provides experience and learnings about incorporating phototherapy into clinical practice in long term care and skilled nursing facilities. Transitioning this clinical and technology experience and expertise into the future tissue treatmetn product applications will be outlined.
RESULTS: This symposium will present the positive effects of phototherapy and the aspects of wound monitoring which are supported by research and are important considerations for clinical practice and product development. Integrating both the use of phototherapy and monitoring wounds are then discussed as it may relate to the development of future innovative products.

Matrix metalloproteinases (MMPs) are a family of at least 25 zinc-dependent enzymes that are collectively able to degrade all extracellular matrix (ECM) proteins. ECM degradation is a normal event in the physiological remodeling associated with growth and maintenance processes such as cell migration, angiogenesis, and tissue turnover. However, the elevated MMP activity in stalled chronic wounds contributes to uncontrolled destruction of connective tissue and increased inflammation. Therefore, targeted reduction of MMP action can reduce tissue degradation, kick-starting the healing process. The clinical understanding of the promise of targeted MMP inactivation therapy for stalled chronic wounds has increased in recent years. The evolving clinical treatment of stalled wounds would clearly benefit from treatment options that are thoroughly researched and have demonstrated positive clinical trial results. We will describe a new MMP inhibiting treatment and demonstrate its unique technology. In addition, we will present clinical trial results of targeted reduction of MMP action that correlates to improved wound condition. This exciting new technology confirms the promise of a unique approach to active wound healing.

Although the use of suction to drain wounds or following surgery has been around for two centuries, modern interest in negative pressure wound therapy (NPWT) was revived with Raffi's 1952 study of negative pressure applied under skin flaps after radical mastectomy. The benefits of NPWT beyond wound draining were revealed in a number of studies conducted in Russia in the 1980s and 1990s. The studies established its role in reducing edema, inflammation and bacterial burden, promoting granulation tissue formation, and reducing wound complications and length of hospital stay. In the US, a gauze-based NPWT technique was used to manage incisional and cutaneous fistulae, whilst in Europe, researchers described the application of foam-based negative pressure of open fractures to produce a clean wound bed and prevent osteomyelitis. Several mechanisms of action have been postulated to account for the effects of NPWT including changes in blood flow and deformation of tissues. There continue to be many unresolved questions regarding the mechanisms of action of NPWT despite many years of its use. The theoretical benefits of NPWT on wound healing mean very little until they can be placed into context. For the wound care specialist, it is what NPWT can do for their patients on a daily basis that matters most. In this symposium, we will take a look at the application of NPWT in a university hospital's acute and critical care surgical floor and wound care clinic over a 24 hour period. Using case studies, there will be examination of which patients and wounds are the best candidates for NPWT, definition of the goals of NPWT and description of common mistakes that lead to the failure or discontinuation of NPWT. There will also be discussion of how to successfully integrate NPWT techniques into clinical practice to improve patient care. This symposium will describe the extensive history of use of NPWT, explore the evidence, old and new, to support the hypotheses related to mechanism of action and discuss the context in which NPWT can be most effectively used.

An estimated 1/3 of patients in the ICU are fecally incontinent. Fecal incontinence is associated with an increase in the risk of nosocomial infections, and the development of perineal dermatitis and pressure ulcers. This symposium will examine the complications associated with diarrhea in the incontinent patient, the implications of the transmission of contaminated stool, and the economic and psychosocial impact on resources, caregivers and patients. Traditional and new methods for containment of stool will be discussed in an effort to identify the most optimal interventions for improving outcomes. Following the presentations, audience members will have the opportunity to interact with faculty during the Ask the Experts panel discussion.

Honey has been used for thousands of years to manage wounds and burns, and has been referred to as "nature's antibiotic". It, like silver dressings, was used fairly regularly up until the advent of penicillin in the 1940's. Even in the modern day there are reported cases of clinicians using honey for challenging wounds, although the cases are mostly anecdotal and there does not seem to be a consistency in reported efficacy. Within the last decade however, researchers have identified one particular species of honey cultivated in New Zealand and Australia - known as Active Leptospermum Honey - which has unique plant-derived components that make this variety well suited for management of wounds and burns in modern times. Not unlike Taxol, one of the most successful chemotherapeutic agents and derived specifically form the Yew Tree indigenous only to the western portion of North America, the plantderived components of Active Leptospermum Honey differentiate it from other honeys in its antimicrobial, anti-inflammatory, immuno-modulatory, and free-radical scavenging activity. The result is a novel product that can be uniquely used to manage all the key aspects of wound bed preparation, and can be used throughout the various phases of wound healing. In this satellite symposium, you will hear two presentations from clinicians that have been involved in some of the key evidence supporting this developing product category. The first presentation will focus on mechanisms of action, and the second will focus on current clinical evidence - notably a recently completed 100+ patient randomized controlled study - for Active Leptospermum Honey and its use in managing hard-to-heal wounds and burns.

Electrical stimulation is an effective adjunctive treatment for wounds that do not heal after an adequate trial of standard therapies. The aim of this symposium is to propose the novel FREMS™ technology, a biocompatible electrical neurostimulation for the treatment of nonhealing wounds. FREMS™ (Frequency Rhythmic Electrical Modulation System) is a composition of electrical signals characterized by negative and multi-modulated pulses which mimic different electrophysiological processes. The impulse sequences interact with biological control systems. FREMS™ is able to improve peripheral cutaneous blood flow, release of angiogenetic factors (VEGF, b-FGF) and reactivate vasomotion activity. These events indicate that FREMS™ can integrate the treatment of non-healing wounds, in terms of healing time and pain control. The symposium will review the experience with existing electrical stimulation treatments, then the focus will be on FREMS™ technology by describing its mechanisms of action and important clinical outcomes. The session will be closed with a discussion on organizational and economical assessment in home treatments.

We need to move from anecdotal dressing selection based on "What worked on Mrs. Jones" to evidence informed practice matching wound components (DIMES) to wound charachteristics. Seventy percent of wound dressings are applied by clinicians who are not experts in wound care. We have a system to select the best dressing and avoid wastage from opening unwanted packages! Deborah Adler saw a problem with patient safety and medication erors. In 2004, it was estimated that 60% of the US population accidentaly took the wrong medication. Motivated by a desire to make peoples' lives easier and safer, Adler designed a comprehensive system for packing and educationg patients as her master's thesis. The result was a completely reinvented prescription bottle and label that revolutionized packing in the US. We have launched this system to improve wound care product selection. Dr. Elizabeth Ayello assesses our current wound care education and proposes strategies for just in time education with a shift from the classroom to point of care where it is more relevant to improve outcomes.

Pain at dressing changes is an issue that has an impact on patient's quality of life. The objective of this session is to raise the awareness of wound related pain in different indications and also to show practical examples of how soft silicone dressings can help minimize pain at dressing related procedures. The session will discuss how pain affects quality of life and healing. Different guidelines and strategies will be highlighted. The session will highlight how pain is managed in three different indications, and provide views on how pain differs between the different indications. Significant demands are being placed on topical therapies, including dressings, which are now expected to fulfil increasingly advanced roles. Important factors for an advanced dressing can include fluid handling, antibacterial protection and maximising patient comfort. This symposium highlights the attributes required of a modern dressing, and discusses the potential advances in the new ALLEVYN* range that have been designed to meet them.
Fluid handling capacity is an essential attribute and it will be suggested that this consists of 3 components, namely absorption (so that unwanted exudate and cellular debris can be removed from the wound), retention (so that enough moisture is retained to optimise healing) and transpiration (so that enough fluid can be removed to keep the dressing comfortable and conformable). The potential for fluid handling offered by the new triple action ALLEVYN technology in this context will be presented and discussed.
Silver is a well established antimicrobial with proven bactericidal effect. Clinical experience with new ALLEVYN Ag will be discussed in relation to those patients whose wounds can benefit from a single dressing that provides both superior fluid management and the ability to provide sustained antibacterial activity for up to 7 days.
It is also important for an advanced dressing range to be able to minimise pain and maximise comfort when treating challenging wounds with varying levels of skin integrity. The inclusion of gel adhesives that provide gentle adhesion to the skin by distributing force over a larger surface area provide this opportunity, and their implications will be reviewed through discussing experiences with ALLEVYN Gentle and ALLEVYN Gentle Border.
No treatment can be assessed independently of the systemic, regional and local factors that influence healing in our patients and this symposium will draw closely on recent case histories that our panel has found to be indicative of the challenges that we are all faced with in advanced wound management today.

The arrival of numerous TNP wound care devices onto the marketplace in recent years has helped facilitate the uptake of this highly effective therapy by clinicians. Inevitably the increased availability of TNP has resulted in a greater variety of wound types and/or patients being treated with this modality. It is important to understand that TNP does not represent a cost-free panacea and as such it is imperative that prior to it being prescribed for a patient, the clinician must give careful consideration to the outcome they wish to achieve and to decide whether this is a realistic goal that can be realised by using TNP. This session will explore some of the issues surrounding cost effective wound care. There will be a specific focus on use of the Exsudex wound drainage system with a range of patients where there are several different outcome goals (including exudate management and wound healing).

What makes a memorable presentation or publication? Why do you listen to certain presenters and glaze over at the very mention of others? Why is it that you read some publications and indeed reference them but ignore others? How does new evidence change your practice? It's all about messaging and delivery. When a piece of communication is to the point, relevant and compelling, it engages - the listener or reader. Engaging people is not magic - it's all about effective communication. There are three steps to effective communication - a solid message, communicating that message with conviction and delivering it in a way that stands out from the crowd. This applies to any message delivered in any format. The content is important but the validation (e.g. statistics) and style (the look) are equally important. The most important tip is to put yourself in the place of the recipient - would you enjoy your message and is it credible. If the answer is yes, then you're on your way to an effective communication and hopefully, changing practice.

Restoring the oxygen balance in chronic and infected wounds
Archimed is an innovative new company focussed on wound healing. New products OXYZYME™ and IODOZYME™ are the first oxygenating moist wound healing systems for chronic and infected wounds.
Both OXYZYME™ and IODOZYME™ are based on a novel technology that uses advanced biochemistry to work like a molecular pump, drawing in oxygen from the atmosphere and transporting it to the surface of the wound, generating iodine in the process. OXYZYME™ delivers oxygen and a low level of iodine to create an oxygenating bacteriostatic environment to encourage healing in chronic and acute wounds. IODOZYME™ also delivers oxygen and a higher, antimicrobial, level of iodine to encourage healing in infected wounds.

The Australian Wound Management Association (AWMA) was formed in 1994 and is a multidisciplinary, non-profit association of people committed to developing and improving wound management for all individuals through education, research, communication and networks.
The Association is linked to the autonomous state wound management associations of New South Wales, Queensland, South Australia, Tasmania, Victoria, Australian Capital Territory and Western Australia. In 2007 the AWMA established a Northern Territory Branch and the Australasian Wound and Tissue Repair Society were formed. There are approximately 3,000 members from the disciplines of nursing, medicine, pharmacy, podiatry, industry and the sciences.
The Association hosted the First World Wound Congress in Melbourne in September 2000. At this meeting the International World Union of Wound Healing Societies was formed. AWMA holds a Biennial Conference hosted by states and Territories on a rotational basis. In 2008 the AWMA hosted the Conference in Darwin NT. The theme of the Conference was "Dreams, Diversity & Disasters" and focused on the influences of traditional and scientific methods on wound management and research. Since November 1993 a quarterly journal (Primary Intention) has been published and is now known as 'Wound Practice and Research'
The Associations aims to improve the community's understanding of wounds and wound management practices, and has developed for practitioners Guidelines for the Prediction and Prevention of Pressure Ulcers (2001). and Australian Standards for Wound Management (2002). A committee is now preparing Venous Leg Ulcer Guidelines for the prevention, diagnosis, treatment and management of Venous Leg Ulcers.

The European Pressure Ulcer Advisory Panel was founded in 1996 'to provide relief of persons suffering from or at risk of pressure ulcers, in particular through research and the education of the public.' It holds annual meetings throughout Europe and in 2008 the 11th annual meeting with the theme 'Risk Management and Early Detection' will be held in Bruges, Belgium from 4-6 September and will be co-hosted by the University of Gent Nursing Science Department. The National Pressure Ulcer Advisory Panel (NPUAP), established in 1987, serves as the authoritative voice for improved patient outcomes in pressure ulcer prevention and treatment through it's three mission committees that focus on public policy, education and research in the United States. NPUAP updated the staging of pressure ulcers and introduced the concept of deep tissue injury. NPUAP more recently has provided input into the MDS 3.0, the CMS CARE Tool, Healthy People 2010, and the IHI 5 Million Lives Campaign; and is collaborating with the EPUAP in developing International Pressure Ulcer Prevention and Treatment Guidelines.
